yeah but if it malfunctioned that it might of popped off and he just short right through it, obviously he would be DOA though.
I think the engine stalled out, if you look at it the wash of the water disappears when it starts to fall, basically indicating that the engines had stopped.

Btw, how do those parachutes open so quickly? I thought it took more time to catch air?

There designed to open with 0 airspeed and 0 altitude. How they got it to work would take some searching.
