Oh so many options and ideas, so little time..
suppose i could strap him to a concrete slab and have a rack of 1000 rusty needles get lowered 1 milameter every hour into him, give him time to recover over night with no needles and then continue everyday until he has some kind of fit, bleeds to death or dies from the rusty infection. while he recovers, only offering him pins to eat and acid to drink, maybe piss and leather handbags if i'm in a good mood that day.
or
for a quick and nasty death, gather all relatives from this awful tragedy and create a huge arena with him in the middle, no where to escape, then unleash thousands of people to rip him to shreds after the entrance to his arena is opened, gladiator style.
im sure i could think of hundreds of painful revenge scenarios to give him but that'll do for now
