Teamspeak is the best that I ever used.

The thing I always felt was missing from the clans I was in was not having a debriefing after a ladder match and discussing what went right and what went wrong. The best things to practice are things that will improve your weaknesses; the best way to identify weaknesses is in a ladder match. I know [WTF] did this and improved very quickly early on.
